UCLA LONG-TERM SERVICES AND SUPPORTS (LTSS) STUDY
INFORMATION SHEET
Dr. Kathryn G. Kietzman at the UCLA Center for Health Policy Research, at the University of California, Los Angeles (UCLA), is conducting a research study on the long-term services and supports needs of Californians who experience difficulties with certain activities of daily life.
PROJECT OBJECTIVE
The purpose of this study is to understand the experiences of Californians who have difficulties with certain activities of daily life, such as dressing, bathing, walking, or doing errands. We want to learn more about the types of services and supports people are aware of, what services or supports they are using, and how well their individual needs for assistance are being met.
Specifically, we will ask you about any informal or formal services or supports you may use to meet your individual day-to-day needs, whether paid or unpaid, and including any help you may get from family or friends. We will also ask how you found out about these services and supports, how effective they are, and how satisfied you are with the quality of assistance you receive.
WHAT WILL HAPPEN IF I TAKE PART IN THIS RESEARCH STUDY?
If you volunteer to participate in this study, we will ask you to answer a 15-minute survey. The survey will ask you about your individual needs for assistance, the amount and type of help you may currently receive, and the extent to which the help you get meets your needs. You will NOT have to answer any questions that you do not want to answer and you can end the survey at any time. The answers you provide in this survey will be linked with the information you provided in the California Health Survey, such as information about your household, health conditions, and experience of well-being. We will keep your individual information confidential. Your other answers will be combined with the answers of other participants and shared with researchers to better understand the health and long term service and support needs of Californians. Only group-level data will be shared via public datasets or publications
HOW LONG WILL I BE IN THE RESEARCH STUDY?
Participation will involve a one-time telephone or online survey of about 15 minutes.
PAYMENT
After completing the survey, you will be provided with a $25 gift card.
ARE THERE ANY POTENTIAL RISKS OR DISCOMFORTS THAT I CAN EXPECT FROM THIS STUDY?
Some survey questions may be about things that are difficult for you to talk about or that might make you sad or upset. You do not have to answer questions that make you uncomfortable. Any private information that you share will be kept confidential.
ARE THERE ANY POTENTIAL BENEFITS IF I PARTICIPATE?
There will be no direct benefit to you from participating in this study. This study will help researchers and policy makers learn more about the needs of Californians who experience difficulties with certain activities of daily life. This information may be used to improve access to services and supports that may help improve the quality of life for Californians like you.
WILL INFORMATION ABOUT ME AND MY PARTICIPATION BE KEPT CONFIDENTIAL?
Any information that is obtained in connection with this study will remain confidential. We will not collect any personally identifying information, such as your name, that could result in your identification.

- Whatever decision you make, there will be no penalty to you, and no loss of benefits to which you were otherwise entitled.
