Alameda County

The Alameda County Health Care Services Agency operates the Health Care Coverage Initiative (HCCI) program known as Alameda County Excellence (ACE). The ACE program was designed to be a self-sustaining model that manages chronic conditions including diabetes, hypertension, congestive heart failure, and asthma by offering full health coverage and services to qualified individuals. ACE is an enhancement to the Alameda County Medically Indigent Services Plan (CMSP). The goal is to improve the quality of service through implementation of the chronic care model including development of integrated medical homes, panel management and improved access to specialty care. Other specific goals include improving specified health outcomes, and lowering high cost episodic emergency services. Services are provided through the CMSP/ACE Provider Network, which includes eight community-based clinics and the Alameda County Medical Center.
