Los Angeles County

The County of Los Angeles Department of Health Services (LAC DHS) participates in the Coverage Initiative through the Healthy Way L.A. Program (HWLA). The Program's goals are to promote high quality care through medical home assignment and a focus on preventative services and primary care. The target population is high-utilizing uninsured patients or those who have been diagnosed with diabetes, asthma, CHF, hypertension or dyslipidemia. Ninety four LAC DHS and Public-Private Provider (PPP) outpatient clinics are participating in HWLA as medical homes.
