Ventura County

The Ventura County Health Care Agency implemented the Health Care Coverage Initiative (HCCI) as the ACE Program in September 2007. The ACE (Access, Coverage, Enrollment) Program focuses on the establishment of a "Medical Home" with an emphasis on prevention and primary care and chronic disease management.

Benefits are comprehensive: Hospital inpatient and outpatient health care services including primary care, adult specialty care, urgent care, behavioral health, dental, prescription drugs and laboratory services. Registered Nurse case management services focus on collaboration, assessment, planning, facilitation and advocacy. The ACE RN case managers are experienced Public Health Nurses who are knowledgeable in assessment, community resources, referrals, case management, and as educators. The chronic disease component of case management involves those ACE Enrollees with diagnoses of Diabetes, Hypertension, Chronic Obstructive Pulmonary Disease, Asthma and Coronary Artery Disease.

In the ACE Program, every enrollee has an established "medical home", benefits including a Registered Dietician, and a sense that they can have access to medical care and prescriptions that will benefit their quality of life.
